Fee Schedule
Gaston Family Practice Pricing
- Per Adult: $140 per month $1,680 annually
- Adult + Children: $160 per month $1,920 annually
- 2 Adults + Children (Family): $300 per month $3,600 annually
*Children are legal dependents 26 and under
There is a one-time registration fee due at the time of sign up, $140 per adult and $20 to add dependents. *Waived if paying for 6 months or more
*Credit Card Payments will have a $3.50 convenience fee per month added to the charge.
Non-Medicare Patients Plan Includes:
We do not file claims with insurance companies, so all patients who are not enrolled in Medicare or a Medicare Advantage plan have access to all the services our office offers:
- Unlimited office visits for adult members, including 1 comprehensive physical yearly
- 6 visits per year per dependent
- Any visits over the allotted number will be 50.00 per visit
- Same day/next day appointments
- Extended office visits
- 24/7 access to an experienced board-certified physician
- All in-office procedures (laceration repair, joint injections, skin lesion treatment, etc.)
- Onsite lab draws with all routine labs at no additional charge
- Tetanus and flu vaccine *if not covered by insurance
Medicare & Medicare Advantage Participants:
Medicare has a Mandatory Claims Submission Rule so to provide our Medicare patients with the best care possible we will file claims with each office visit. For our Medicare patients, the monthly service fee will cover:
- Copays, coinsurance, and deductibles for services at our office
- 1 comprehensive physical yearly
- Same day/next day appointments
- Extended office visits
- 24/7 access to an experienced board-certified physician
- All in-office procedures not covered by Medicare
- Ear wax removal
- Skin tag removal
- Cryotherapy for non-cancerous lesions
- Surgical removal of benign skin lesions
- Toenail removal/care
- Screening EKG
- Nutritional counseling
- Tdap vaccine *if not covered by insurance
